1. Direct
Deposit - If the check is directly deposited with a
financial institution, the Change of Address form can be
completed using the online form found under the “PIN”
link.
- Paper Check – If the check is mailed, the Change
of Address form may be filled out, printed, signed
and mailed to LASERS. Retirees may still contact
LASERS to have a Change of Address form mailed to
them. Once received, the form may be completed with
an original signature and mailed to LASERS. LASERS
does require the retiree's signature before an
address will be changed. If the retiree's signature
has changed because of health reasons, the retiree
must sign the form in the presence of a:
- LASERS Retirement Benefit Analyst, or
- Notary Public.
Changes of addresses received by LASERS prior to the
15th of the month will be effective on the next monthly
benefit check.
